N conjunction with the Time Slips Project, project director Anne Basting is now offering free, educational presentations throughout the spring. Time Slips is a public arts project whose aim is to encourage creative expression among people with Alzheimer's disease and related dementias. What's amazing about the Time Slips Project is that it touches the fields of health sciences and the arts and humanities, says Basting. I have done presentations for everyone from nurses to acting students. Basting's presentations feature slides chronicling the project and address issues of memory and identity, the value of creative selfexpression, structures of autobiography, and innovative, hands-on storytelling techniques. In the first year two years of the project, Basting tested a storytelling technique and translated the stories told by people with dementia into a series of public arts events in Milwaukee, Wisconsin. Now in the third year of the project, Time Slips will produce the arts and educational events here in New York, including a website timeslips ; , a play production, an art installation, and a roundtable discussion entitled: Is There an Art to Dementia Care? The arts events will begin in October and run through November, 2001. Pain is a common symptom in MS. Those who experience pain feel its effects on their daily life activities, such as work and recreation, and in their mood and enjoyment of life. The fact that a record number of people responded to our online survey results on page 24-25 ; for this edition of the magazine shows how important this problem is for the many people who experience pain. Alarmingly, 90 percent of people with MS said they were not completely relieved of pain with treatment. This discrepancy highlights an area that demands attention from health care professionals and researchers. Many clinical features of pain are often unrecognised by clinicians and it may be difficult for many people with MS to find the right words to describe the pain they experience. At the same time, scales for evaluating pain rely on subjective reports and may be biased by any combination of circumstances, making the evaluation of pain severity and its impact on daily living difficult. Pain is often present with other symptoms, such as depression, spasticity and mobility difficulties, underlining the importance of addressing MS comprehensively. The situation is complicated further by the fact that a person with MS may also have other illnesses, making the diagnosis and treatment of pain very complex. Another concern is that in some cases the side effects of pain medication can be as distressing as the pain itself and therefore cannot be used at an optimal level. Considering it has a significant influence on a person's quality of life, unmanaged or ineffectively managed pain can have consequences that go beyond issues of symptom management. Therefore, involvement of the MS team, including the person with MS and his or her family, is fundamental for the identification, assessment and optimal treatment of pain. We hope that this edition of MS in focus will clarify some issues related to the diagnosis and management of pain and be a useful resource, particularly for health professionals who may not realise the impact pain has. The meat was then sent abroad for human consumption. In addition, thousands of horses were shipped live to Canada for slaughter there. The majority of horses slaughtered in these plants are not old, sick or lame but instead are young, healthy horses bought by "killer buyers" who attend horse auctions where they compete with families and other horse brokers looking for good, sound horses. The transport of the horses to the slaughter houses is often extremely inhumane. These double decker trailers are overcrowded and many horses are severely injured even before arriving at the plant. Some are shipped for more than 24 hours at a time without food, water or rest. In addition, the slaughter method used to kill these horses is inhumane. The horses often endure repeated stuns or blows and remain conscious during their slaughter. Americans do not eat horses. We do not raise them for food. This is surprising considering that the interaction between the primary amine and Asp1133.32 makes the strongest contribution to the binding energy. The metal non-electrostatic VHC VORTEX PARI ; has been designed to overcome these challenges. In this study we analysed the performance of the VORTEX , in comparison to a plastic VHC Aerochamber Plus , Trudell ; and to pressurized metered dose inhalers pMDI ; alone. Methods: Tests were performed with three different brands of CFC-pMDIs respectively: beclomethasone, salbutamol and ipratropium as active agents. Respirable mass RM, particles: 5.8 m ; and mass median aerodynamic diameter MMAD ; were determined by an Anderson cascade impactor at 28.3 l min in accordance with the FDA 510 k ; regulations using gravimetric analysis. Three trials for each drug were performed. Results: In all tests, the measured MMAD with the VORTEX were similar to the Aerochamber Plus or the pMDI alone. By contrast VORTEX delivered in all cases a higher RM than Aerochamber Plus or the pMDI alone. For example, VORTEX provided up to a 73% higher RM of an ipratropium-MDI Afrovent , Boehringer Ingelheim ; than Aerochamber Plus . Conclusion: This study indicates that the non-electrostatic VORTEX has the potential for a better therapeutic effect especially for the poorly coordinated patient.

It all makes for a stimulating, thought-provoking polemic, ending with important chapters by each of the editors and arguing for the prime place for narrative within clinical practice in a working relationship with epidemiology and scientific research. But I wish they had given more space to how you teach all this to young doctors trying to work under intolerable time pressure. HYPOSPADIAS is a congenital abnormality of genital development that occurs in about one in 200 boys. Failure of fusion of the inner and outer genital folds during embryogenesis results in the urethral opening failing to be shifted from the normal female position in the perineum to the tip of the penis. In addition, there is failure of the foreskin to cover over the ventral surface of the glans, leading to what is known as a dorsal hood of foreskin figure 7 ; . The third major abnormality in hypospadias is caused by the relative deficiency of growth on the ventral surface around the urethra. This leads to a relatively shorter ventral surface of the penis from the dorsal surface, so that on erection, the penis bends, known as chordee. If there are other abnormalities of the external genitalia, including failure of the testes to descend normally and or part of the scrotum to fuse, the child should be assumed to have ambiguous genitalia and referred immediately at birth for investigation. This should not be delayed until later in infancy, as might referral for treatment of simple hypospadias. The indications for surgical treatment relate to both the functional and cosmetic problems. Downward deflection of the urinary stream secondary to the ventrally displaced meatus leads to inability to void standing up and discrimination, particularly in primary school. Chordee on erection leads to inadequate sexual function as an adult. The aims of surgery are to correct the chordee, get the urethral meatus to the tip of the penis, provide a good cosmetic appearance and avoid complications. Penile reconstruction for hypospadias is challenging and is usually done by a small group of well-trained paediatric surgeons and paediatric urologists. Most paediatric hospital surgical departments can advise on the surgeons familiar with these operations. The operation may be done in one or two stages, depending on the surgeon's own preference and the degree of difficulty of the abnormality. The main complications are failure of healing, with wound infection, or complete wound breakdown and development of a urinary fistula. In addition, strictures or meatal stenosis may inhibit the urinary stream. Poorly corrected chordee is one of the serious complications, as this may not be appreciated until later in life, when it may be more difficult to correct. The incidence of complications of hypospadias surgery varies depending on surgical experience and degree of complexity, but is about 10 to 20%. The prognosis for future adult sexual function depends on the original severity of the abnormality. Boys with very severe hypospadias, particularly if associated with a small penis, may suffer significant psychological problems in adolescence and early adult life. However, most boys with hypospadias should have a penis of acceptable appearance and relatively normal function after surgical correction and biaxin!
